Application Requirements
Academic Requirements
- Minimum GPA: 3.0 on the OMSAS Scale.
- MCAT Requirements: Minimum thresholds are Total Score: 500, and 125 in each section (Biological and Biochemical Foundations, Chemical and Physical Foundations, Psychological/Social/Biological Foundations, and Critical Analysis and Reasoning Skills). All thresholds must be met in a single test. Queen's will assess based on your test result that meets the minimum thresholds.
- Prerequisite Courses: Queen's does not have strict prerequisite courses. You are allowed to apply in third year, meaning you need to have 90 credit-hours or 15 Full Course Equivalents by the end of your application cycle.
- Casper: Queens does not disclose a Casper cutoff. The threshold is determined annually and not disclosed. Most applicants aim for strong performance to remain competitive.
The New Lottery System (QARS)
Queen's Medicine uses a lottery system referred to as the Qualified Applicant Randomization Selection (QARS) as part of its admissions process. This system was designed to increase fairness and equity among applicants.
- How It Works: After meeting the academic and Casper requirements, applicants are entered into a lottery system to randomly select candidates for MMI interviews, reducing subjective evaluations and promoting equal opportunity.
- Final Admission Decision: Following the MMI, approximately 50% of interviewees receive an invitation to a Panel Interview. After the Panel interviews, Queen's performs a full file review of the ABS entries and the CAF (Confidential Assessment Forms/references). The exact factors and weights used remain unclear.

Queens has specialized application pathways that you may be eligible for. These include:

The Interview
Interview 1 Format - MMI: Queen's uses a Multiple Mini Interview (MMI) format, conducted virtually for the 2025-2026 cycle.
Queen's MMI is asynchronous. You have 2 minutes to read the initial prompt and 5 minutes to answer. You are allowed to take notes and consult them if you wish, though doing so may impact your delivery to the camera. There are usually follow-up questions as well, with a shorter amount of time for thinking and a shorter response.
It has been rumoured that the MMI format has been changed (2025-2026) to not allow any preparation time, meaning your recorded response begins right after the question is read aloud.
The MMI assesses non-cognitive qualities including empathy, critical thinking, ethical decision-making, and communication skills. Your performance in the MMI will largely impact your chances of receiving a Panel interview invite.
Interview 2 Format - Panel: Approximately 50% of MMI interviewees receive an invitation to a Panel Interview near the end of February, a more personal assessment conducted with two interviewers. This is a deeper exploration of your experiences, motivations, and suitability for Queen's MD program. Final decisions may be based on a more holistic assessment of your ABS and MMI/Panel interviews.
